Who we are
Franklin Bright Smiles operates a dental practice at 3/54 Nullarbor Avenue, Franklin ACT 2913. This policy explains how we handle personal information, including health information, in line with the Privacy Act 1988 (Cth) and the Australian Privacy Principles.
What we collect
We collect only what we need to provide safe dental care and to run the practice.
- Contact details: name, address, phone number and email address
- Health information: dental and medical history, medications, allergies, clinical notes, images and X-rays
- Payment information: health fund membership, Medicare or DVA details, and billing records
- Website information: pages viewed and general usage data, where analytics is in use
How we use it
To assess, plan and provide dental treatment; to process health fund, CDBS and DVA claims; to send appointment reminders; to meet our legal and professional record-keeping obligations; and to respond when you contact us.
We do not sell personal information, and we do not send health information, symptoms or treatment selections to advertising platforms.
Who we share it with
Only where necessary: other treating practitioners with your consent, dental laboratories and specialists involved in your care, health funds and government agencies for claims, our practice software and secure IT providers, and anyone else where the law requires it.
Website forms and enquiries
Contact and callback forms ask only for the details needed to call you back. Please don't include symptoms or other health information in a web form. A callback request is not a confirmed appointment.
Storage and security
Records are held in secure practice software with access limited to authorised staff. Health records are retained for the period required by law and professional obligations.
Access, correction and complaints
You can request access to your records or ask us to correct them by contacting the practice on (02) 6274 0404 or at reception@franklinbrightsmiles.com.au. If you're not satisfied with our response, you can contact the Office of the Australian Information Commissioner.
